Supporting Your Family Member's Recovery with Home-Based Physiotherapy
Is a family member preparing to come home from hospital? Are you worried about how they will manage, whether they will regain their independence, or simply whether they are getting the right support?
Home Links Physiotherapy is a community physiotherapy service covering South East London and Kent. We specialise in supporting older people and those with complex rehabilitation needs — visiting them at home, at their pace, in familiar and comfortable surroundings.
Why home-based physiotherapy makes such a difference
The period following a hospital discharge is often one of the most critical in a person's recovery. Returning home can feel daunting — and for many people, the gap between leaving hospital and regaining confidence and independence is where things can go wrong.
Home-based physiotherapy bridges that gap. Rather than asking your family member to travel to appointments when they are at their most vulnerable, we come to them — working within their home environment to rebuild strength, mobility, and confidence where it matters most.

What Home Links offers your family
Specialist expertise in post-hospital rehabilitation Our physiotherapists are experienced community clinicians, not generalists. We understand the specific challenges of recovering from hospital stays — whether following surgery, a fall, a stroke, or acute illness — and we know how to support people safely through that process.
Personalised assessment and care plans We begin with a thorough assessment of your family member's needs, goals, and home environment. From this, we develop a personalised rehabilitation plan focused on what they want to achieve — whether that is getting up the stairs independently, returning to the garden, or simply feeling steady on their feet again.
Coordination with NHS and other health professionals Many of our clients are on NHS waiting lists or have recently been discharged from NHS care. We are experienced at working alongside NHS colleagues and other health and social care professionals, slotting in wherever we are most needed — whether that is topping up NHS provision, filling a gap in the waiting list, or continuing rehabilitation once statutory support has ended.
Involving the family We recognise that caring for a family member can be as exhausting as it is worrying. With your family member's consent, we are happy to involve family members in updates, progress conversations, and decision-making. We can also provide carers with practical strategies and exercises to support recovery between our visits.
Absolute respect for consent and confidentiality The dignity and privacy of every client is central to everything we do. We will always work within your family member's wishes and at a pace that is right for them.
How to get started
Getting support from Home Links is straightforward:
Get in touch — call or email us and tell us a little about your family member's situation. We will talk through their needs and explain how we can help.
Initial assessment — one of our physiotherapists will visit to carry out a comprehensive assessment and begin developing a personalised plan.
Regular home visits — your physiotherapist will work with your family member in their own home, building their programme over time and adjusting it as they progress.
There is no need to wait until things reach a crisis point. The sooner rehabilitation begins, the better the outcomes tend to be.
